Every figure on this page is computed from the inputs you enter, by the method stated below it. The only constants HeadcountDesk supplies are the published federal payroll tax rates and the overtime multiplier, each cited where it is used. The defaults are a worked example to replace with your own.
A new hire questionnaire collects, before day one, the facts the employer needs and will otherwise chase: legal name and contact details, emergency contact, the tax withholding forms, bank details for pay, the I-9 document choice so day one's verification is ready, equipment and access needs, and any accommodation the employer should arrange. It is not a personality quiz; every question maps to a checklist item. The onboarding checklist on this site carries it, free and without an account; HeadcountDesk Pro sends it and files the answers against the employee.

The identity and contact block
Legal name as it appears on the I-9 documents, address for the state forms, phone and personal email for the days before the work account exists, and the emergency contact with their relationship.
The pay and tax block
The W-4 and the state withholding form, the bank details for direct deposit, and the I-9 document choice named in advance so the day-one verification is a formality rather than a scramble.
The setup block
Equipment preferences where the business offers a choice, software and access the role needs, and any accommodation to have ready. Asking before day one is what makes day one work.
Where the answers go
Each answer lands on its checklist item and in the employee's file, and the sensitive ones live with the confidential material. Pro routes them; the free checklist lists them. From the vendors' own pricing pages: Homebase lists Essentials at $24 per location per month billed annually or $30 monthly, with a free Basic plan for one location of up to ten employees; BambooHR lists Core at $10 and Pro at $17 per employee per month; Deputy lists Core at $6.50 per user per month. HeadcountDesk Pro is $24 a month per location.
