An employee change form records one change to the employee's record with a date, a reason and an approval: a new address or bank account, a change of role or manager, a pay change, a change of hours or status, a name change. It is the document that keeps the payroll, the directory and the file in agreement, and the history of the forms is the history of the employment. What the form holds
Employee, the field changing, the old value, the new value, the effective date, the reason, the requester, the approver. One change per form so the history is readable.
Who approves what
An address change is the employee's to make; a pay change is the owner's to approve. The handbook sets the rule; Pro routes the form to the right approver.
The record updated, the history kept
The current values feed payroll and the directory; the old values stay in the history with the form that changed them. That history is the answer to any later question about when pay changed and who agreed.

From the vendors' own pricing pages: Homebase lists Essentials at $24 per location per month billed annually or $30 monthly, with a free Basic plan for one location of up to ten employees; BambooHR lists Core at $10 and Pro at $17 per employee per month; Deputy lists Core at $6.50 per user per month.
